Trick 1

It's not easy when someone close to you is getting ready to leave this earth. At times like these you and your loved ones can use as much help as possible.

This non-profit provides a resource guide and toolkit that are excellent starting points. They provide ideas, inspiration and information on thorough and effective end-of-life preparation. You will also get help with your priorities along with help so your loved ones can clarify and honor your end-of-life preferences.

A values worksheet offers questions to consider when making decisions and preparing documents concerning healthcare preferences. There are also forms to inform your physician or other care provider of your consent to or refusal of certain specific therapies.
In addition, you will also get help preparing an advance directive which outlines end-of-life priorities ("who will speak for me")
